Registration and Information

  • Signing Up - School Year
    1. Sign up online and reserve your child’s spot Visit discoverDYCD to submit online registration 2. Once you’ve submitted your information online, you’ll receive an email confirmation that your application was submitted. 3. A program team member will then review your application’s eligibility and contact you to confirm interest and collect additional information. 3. Your child’s full application packet, including medical forms, are due before the start of the program. The medical form must be completed by a physician and include signature, date of exam, and stamp. 4. If you wish to sign up in person, come to one of our local ASPIRA programs and we will be happy to assist you.

Once you’ve submitted your information online, you’ll receive an email confirmation that your application was submitted. 3. A program team member will then review your application’s eligibility and contact you to confirm interest and collect additional information. 3. Your child’s full application packet, including medical forms, are due before the start of the program. The medical form must be completed by a physician and include signature, date of exam, and stamp. 4. You must complete and return the Summer Rising Supplemental Form in order to complete your registration. 5. If you wish to sign up in person, come to one of our local ASPIRA programs and we will be happy to assist you.
  • Required Paperwork
    According to the NYC Department of Health, all participants must have a completed medical form (must be one year from the date of issue) prior to the start of the program. It’s also a good idea to keep extra copies of the medical form for your records. If your child does not have a complete medical form along with the health care practitioner's stamp or signature, they may not attend. During the registration process, it is extremely important that families notify the program if the child has any medical conditions such as, but not limited to, allergies, asthma, diabetes or other conditions. Additionally, you must submit all necessary consent forms, including but not limited to: Consent for Emergency Medical Treatment Consent to Participate Authorized Pick Up Authorized Emergency Contact Consent for Photography/Videotaping and Use of Original Work Consent to Make Referrals and Share Information
